Health Declaration
A valid health declaration is required to apply for a practical exam at the CBR.
How to apply?
You apply for the health declaration yourself via mijn.cbr.nl. This is separate from the driving school — it is your own responsibility.
Log in to mijn.cbr.nl with your DigiD
Go to "Gezondheidsverklaring aanvragen" (Request health declaration)
Answer the questions honestly
Submit the declaration in good time so the CBR can process it before your exam
Costs
The health declaration costs €45.25 and is processed directly by the CBR. This amount is not included in our driving packages.
Good news for autism and ADHD
From 1 December 2025, people with ADHD or autism are no longer referred for additional medical examinations. AD(H)D and autism are expected to be fully removed from the Health Declaration on 1 April 2026.
